What kind of holster, was retention adjusted?

Just curious was it in a decocked state?

A drop safety should work regardless. I witnessed a chambered PPQ being dropped with no adverse consequences.
While I feel sorry for a fellow Walther owner being injured, I am also sure that we don't know some of the material facts on the general condition of the firearm and any work done on it.
